Saint John is the oldest incorporated city in Canada, chartered in 1785, and it sits on the Bay of Fundy where the tides run about eight metres. That tide is high enough to force the river backwards through a gorge in the middle of the city twice a day. The port is the third largest in Canada by tonnage and the Irving refinery is the country's largest, running over 320,000 barrels a day.

The city lost about 10,800 residents between 1981 and 2001 and then started growing again after 2011, driven almost entirely by immigration. The uptown that survived is unusual: after the Great Fire of 1877 it was rebuilt in brick and stone rather than wood, which is why there is a dense run of Victorian and Second Empire architecture around the City Market, itself operating since 1876.

Anything specialized in southern New Brunswick has generally meant a drive. Fredericton is an hour and a quarter, Moncton an hour and a half, Halifax over four hours, and the nearest studio devoted to removal is about fifty minutes away in Sussex. Removal is a course of treatment spread over a year or more, and that distance is the reason a lot of people never start.
